The St. Croix County Clerk’s office accepts passport applications BY APPOINTMENT ONLY.

The St. Croix County Clerk’s office is a designated acceptance facility for obtaining a U.S Passport. You will not receive your passport from us. Our role is to accept completed passport applications and fees for processing. We cannot accept passport renewal forms.

It is imperative applicants have completed their applications, provide all required documentation and payment when applying. If applicants appear without necessary documents, it may be necessary to reschedule the appointment.

All applicants must appear in person (including minors). Minors under the age of 16 must appear with both parents. If one parent is not available, a DS-3053 form or a notarized authorization from the absent parent and a copy of his/her identification (front and back) is required. Minors age 16 and 17 should appear with one parent.

If applicants do not speak English, a third party needs to be present to interpret at the time of application.

Method of Payment

We can only accept checks or money orders to attach to passport applications made payable to U.S Department of State. A check or money order needs to be attached to each application submitted. A separate payment for the execution and photo fees paid to the County Clerk may be check, money order or cash. For information regarding fees go the U.S Department of State website

We take passport photos on-site for $10 per applicant (cash or check only). Please Note: All applicants must appear in person.
