St. Croix County Public Health works with community partners and residents to better understand the health of the community. This process, called a Community Health Assessment (CHA), gathers input from County residents through a community health survey and community conversations. Additionally, St. Croix County data was gathered to produce a data dashboard on factors that affect our community’s health.
The information collected through the CHA guides our efforts and helps shape the Community Health Improvement Plan (CHIP). The CHIP serves as a roadmap for coordinating resources to create lasting positive change in the community as it pertains to our identified health priorities.
The Community Health Improvement Plan (CHIP) will be available in early 2026. See below on how to get involved with the CHIP.

The Healthier Together-Pierce and St. Croix County coalition (commonly known as Healthier Together) is a collaboration between St. Croix and Pierce County community members and organizations with a goal of supporting community health improvement efforts. We do this by providing opportunities for networking, information and resource sharing, and collaboration on strategic initiatives that improve the health of Pierce & St. Croix communities.
Historically, through direct implementation efforts of the Healthier Together Coalition, St. Croix and Pierce County completed a joint CHA and CHIP. However, the needs of our counties and leadership organizations have evolved, so moving forward from the 2023-2025 CHA CHIP cycle, each county will develop and implement separate community assessments and improvement plans.
Healthier Together has meetings 3x/year, shares regular updates between meetings, and welcomes new members.
